Yes. ATM. You see, tried a 4 tick SL. Entry was filled but the SL was not able to be entered into the exchange when the price went beyond the SL.

Right, it's possible on fast markets with a very tight stops. If the price "went beyond", BUY/SELL STOP or BUY/SELL STOP LIMIT order cannot be placed. Simply try to place BUY/SELL STOP below/above the market price manually by pressing middle mouse button. You'll also get rejection from the trading server. You need change your DOM settings "Use stop market for stop loss orders" to TRUE. But be ready for 0.01 - 0.04 slippage.
