The Uniform Policy Provisions, Health Insurance (UPPH) are a set of standard contract provisions that apply to all health insurance policies sold in the United States. The UPPH were developed by the National Association of Insurance Commissioners (NAIC) in an effort to make health insurance policies more understandable and comparable for consumers.

The UPPH cover a wide range of topics, including:

• Claims procedures
• Renewal and termination

The UPPH are not a substitute for reading your own health insurance policy. However, they can help you to understand the basic terms and conditions of your policy.
